We have a new name and look, but our care and promise to patients and our community is the same

We are excited to announce that Heartland Health Centers has changed its name to Tapestry 360 Health. The name change will help establish our unique identity from other similarly named organizations.

Please know that our new name will NOT affect your care in any way. We are the same organization with the same services, providers, care teams and leadership. You will still have a choice of primary care provider and access to quality affordable care.

If you are an established patient, it won’t affect your choice of primary care provider or your access to quality and affordable care for every age and stage of life. We are the same organization with the same great providers and care teams under the same leadership and board.

• Our new name represents the beautiful tapestry of the communities we serve, a blend of patients, staff, and community partners inclusive of all races, ethnicities, cultures, religions, socio-economic backgrounds, and orientations woven together.
• “360 Health” affirms our dedication to comprehensive healthcare for all.
• We promise to work Together for Better Health – with you, and our healthcare and community partners, to improve your whole health.

Please note that while we are now Tapestry 360 Health, you may continue to see the Heartland Health Centers name on your medical documents while we change all our systems to the new name.
