Who We Are
Heartland Health Centers is a federally qualified health center with 18 locations serving Chicago’s north side and nearby suburbs. In 2020, we provided 27,000 patients with a medical home where we offer affordable and comprehensive primary care, oral health care, and mental health care services. As a medical home, HHC ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and oriented to addressing all the health care needs of each patient.
What is an FQHC?
A Federally Qualified Health Center (FQHC) must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.
Serving more than 28 million patients nationally, FQHC’s receives federal funding and enhanced reimbursement from Medicare and Medicaid.
We’re open to the entire community—the uninsured as well as those who have Medicaid, Medicare, or private insurance. We offer medication discounts and free interpretation services in over 35 languages.
Quality Leader & Joint Commission Accreditation & Certification
Heartland Health Centers has achieved Joint Commission Accreditation for Ambulatory Care, as well as certification as a Primary Care Medical Home (PCMH).
In 2021, we were also recognized as a Primary Care Medical Home, PCMH by HRSA as a leader in the movement to provide healthcare that ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and orientated to addressing all the health care needs of each patient.
The Health Resources and Services Administration recognizes Heartland Health Centers as a COVID-19 Data Reporter as one of about 400 federally qualified health centers nationally that have supported data gathering for public health emergencies.
We invest in support for our medical providers and staff because we believe that a well-supported staff is an essential part of delivering high-quality care to our patients and community. In September 2021 the American Medical Association recognized Heartland Health Centers with a Joy in Medicine Award for health care organizations that have committed to efforts that improve medical provider satisfaction and reduce burnout.
Our integrated primary care and behavioral health partnership with Trilogy Behavioral Health Care is nationally recognized, winning the reintegration Award for Clinical Medicine from the National Council for Behavioral Health in 2012.
Heartland Health Centers was recognized for our newest health center at 1300 W. Devon. The Urban Land Institute awarded us with their Vision Award for Catalytic Development, and the Rogers Park Business Alliance gave us their Capital Improvement Award. Our Devon center is LEED Platinum, the highest environmentally sustainable distinction, and the only health center in Illinois with this distinction.
Heartland Health Centers is a HHS Health Center Program grantee under 42 USC 254b, and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
