Who We Are
Heartland Health Centers is a federally qualified health center with 15 locations serving Chicago’s north side and nearby suburbs. In 2016, we provided 21, 000 patients with a medical home where we offer affordable and comprehensive primary care, oral health care, and mental health care services. As a medical home, HHC ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and orientated to addressing all the health care needs of each patient.
What is an FQHC?
A Federally Qualified Health Center (FQHC) must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.
Serving more than 18 million patients nationally, FQHC’s receives federal funding and enhanced reimbursement from Medicare and Medicaid.
We’re open to the entire community—the uninsured as well as those who have Medicaid, Medicare, or private insurance. We offer medication discounts and free interpretation services in over 35 languages.
Awards and Distinctions
Our integrated primary care and behavioral health partnership with Trilogy Behavioral Health Care is nationally recognized, winning the reintegration Award for Clinical Medicine from the National Council for Behavioral Health in 2012.
Heartland Health Centers was recognized for our newest health center at 1300 W. Devon. The Urban Land Institute awarded us with their Vision Award for Catalytic Development, and the Rogers Park Business Alliance gave us their Capital Improvement Award. Our Devon center is LEED Platinum, the highest environmentally sustainable distinction, and the only health center in Illinois with this distinction.
An Affiliate of Heartland Alliance
Heartland Health Centers was founded in 1993 and originated within Heartland Alliance for Human Needs & Human Rights, the leading anti-poverty organization in the Midwest. We remain an affiliate of Heartland Alliance but in 2005 we became incorporated and an independent non-profit organization. Like our founding organization, we are passionate in our view that healthcare is a basic human right.
Heartland Health Centers is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). Heartland Health Centers is a FTCA deeming facility. This allows Heartland Health Centers to service all patients under the protection of the Federal Tort Claim Act.
