In 2021, we were also recognized as a Primary Care Medical Home, PCMH by HRSA as a leader in the movement to provide healthcare that ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and orientated to addressing all the health care needs of each patient.

The Health Resources and Services Administration recognizes Tapestry 360 Health as a COVID-19 Data Reporter as one of about 400 federally qualified health centers nationally that  have supported data gathering for public health emergencies.

We invest in support for our medical providers and staff because we believe that a well-supported staff is an essential part of delivering high-quality care to our patients and community. In September 2021 the American Medical Association recognized Tapestry 360 Health with a Joy in Medicine Award for health care organizations that have committed to efforts that improve medical provider satisfaction and reduce burnout.

The Health Resources and Services Administration recognizes Tapestry 360 Health as a Access Enhancer as one of about 400 federally qualified health centers nationally that support removing barriers to promote health equity.

In 2023 the HRSA recognized Tapestry 360 Health with a Health Disparities Reducer Award for health care organizations that have met targets for all racial or ethnic groups they served within the latest reporting year.

In 2023 HRSA recognized Tapestry 360 Health with a Public Health Champion Award for health centers providing COVID-19 vaccinations or testing to the largest percentage of patients.

Federally Qualified Health Center (FQHC)

Tapestry 360 Health is a Federally Qualified Health Center (FQHC). A Federally Qualified Health Center (FQHC) must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors.

Serving more than 28 million patients nationally, FQHC’s receives federal funding and enhanced reimbursement from Medicare and Medicaid.

Tapestry 360 Health patients receive equal access to polite and respectful care regardless of race, ethnicity, color, gender, national origin, disability, religion, sexual orientation, gender identity, age or ability to pay.
