Heartland Health Centers has a sliding-fee scale for those that do not have health insurance.

Patients will pay a discounted fee based on household size and income. This fee will include the visit with the provider, x-rays and labs, if ordered.

Proof of income for the past 30 days will be required at your first visit. If you have no income, a notarized letter from the person supporting you will be required.

If you are prescribed medication, our staff will work with you to obtain medicine at an affordable cost.

If you need specialized care, our staff will refer you to one of our partner hospitals depending on the service needed. Payment for specialized service will depend on your income and household size as well; some services may be free.
