Uninsured? Start here

Heartland Health Centers has a sliding-fee scale for those that do not have health insurance.

This fee will include the visit with the provider, x-rays and labs, if ordered. The sliding-fee scale is based on household size and income. Proof of income will be required at your first visit. If you have no income, a notarized letter from the person supporting you will be required. If you are prescribed medication, our staff will work with you to obtain medicine at an affordable cost.

If you are interested in becoming a Heartland Health Center patient call our scheduling department at (773) 751-7850.
