Aesha Patel is a Family Nurse Practitioner and the Care Delivery Innovation Lead at Heartland Health Centers. She has been a part of the HHC family for 5 ½ years. Prior to joining the Heartland team, Aesha graduated with a Doctor of Nursing Practice (DNP) from Rush University. After a few years of practicing primary care, she discovered her passion for patient-centered design, healthcare informatics, and innovation. She realized the importance of a strong clinician presence in the building and implementation process of healthcare technology, and went on to receive a Post Masters in Healthcare Informatics Certificate from UIC.
Aesha is most driven by her patients and her team, on a tireless mission to redesign primary care using a patient-centered approach. She helped facilitate the training and onboarding of the newly created Care Team Coordinator (CTC) role as well as the implementation of the advanced team-based care (aTBC) model in 2019 during ICAP’s launch; she is now leading the reimplementation of aTBC to better understand how to optimize this model in the telehealth environment while also sustaining it for in-person care.
She is fluent in Spanish and has a basic understanding of Gujurati.
